Accident Summary:

 ON THE 26TH OF JUNE 1969, WO1 WILLIAM W. CARROW (AC) AND WO1 ROBERT L. DUNLAP (P) WERE FLYING AN AUTHORIZED RESUPPLY MISSION IN THE KONTUM DAK TO AREA. AFTER PASSING THROUGH THE POLEI LKENG PASS THEY OBSERVED BOTH OF THE LZ'S AND NOTED THAT WEATHER CONDITIONS AT BOTH WOULD NOT PERMIT A LANDING. THEY ELECTED TO LEAVE THE CARGO AT POLEI LKENG FOR LATER DISTRIBUTION. THEY RETURNED TO MARY LOU FOR A SECOND AIRCRAFT COMMANDER DECLINED TO REFUEL. THEY DEPARTED MARY LOU AND FLEW A LOAD TO LZ JUDE AND RETURNED TO POLEI KLENG TO DISTRIBUTE THE EARLIER CARGO. THEY THEN RETURNED TO POLEI KLENG WHERE THEY LAODED FOUR PASSENGERS AND A SMALL AMOUNT OF BACK HAUL AND DEPARTED FROM MARY LOU. APPROXIMATELY TEN MINUTES OUT THE 20 MINUTE FUEL WARNING LIGHT CAME ON. AT THIS TIME, THE FUEL GAUGE READ THREE HUNDRED POUNDS. APPROXIMATELY THREE MINUTES LATER THE RIGHT FUEL BOOST PUMP CAUTION LIGHT CAME ON. FUEL PRESSURE FLUCTUATED BETWEEN FIFTEEN AND TWENTY-FIVE POUNDS, DROPPED TO ZERO AND RPM BEGAN TO DECAY. MR. CARROW, DID A 180 DEG TURN TO SET HIS APPROACH UP INTO THE WIND. AT THE LAST POSSIBLE MINUTE, MR. CARROW DECIDED TO DA A RIGHT PEDAL TURN TOLAND PARALLEL TO A DIKE. PHOTO#1 SHOWS THE FIRST POINT OF IMPACT WHERE TH STINGER AND A PORTION OF THE TAIL BOOM STRUCK THE DIKE IN SUCH A WAY AS TO INDICATE A HEADING OF APPROXIMATELY 300 DEGREES AND A RIGHT PEDAL TURN. THE TURN IS EVIDENCED BY THE PERFECT IMPRESSION OF THE STINGER AS IT SLAMMED THE DIKE TRAVELING FROM RIGHT TO LEFT. PHOTO #2 ALSO VERIFIES THIS BY THE DIRECTION IN WHICH THE STINGER IS BENT. PHOTO #3 SHOWS THE STRIKE AND THE FINAL RESTING PLACE OF THE AIRCRAFT. PHOTOS #4 & #5 SHOW THAT THE MAIN ROTOR BLADES STRUCK THE PADDY CAUSING THE AIRCRAFT TO SPIN APPROXIMATELY 210 DEGREES TO THE RIGHT AND LAND LEFT SIDE LOW SLIDING TO THE LEFT. AS THE AIRPLANE SLID, THE LEFT SKID CROSS TUBE BOTH FORE AND AFT FAILED CAUSING THE AIRCRAFT TO ROLL ON ITS LEFT SIDE.\\
